We are currently accepting patients for each of our departments; Medical, Dental, and Behavioral Health. All new patients must complete our Patient Registration Packet prior to their first visit. You may download and print the packet from our website or arrive 30 minutes before your appointment and complete the new patient registration process. Items you need to bring to your first appointment would be:

If you are Native American such as, grandparents Indian Verification, parents Indian Verification, parents County Birth Certificate and County of Birth, your own Birth Certificate.
