Pit River Health Service, Inc.
offers Native American patients the services of Angela Diaz, who is our
child and family social service and community referral person. She can
assist in helping tribal members obtain alcohol or substance abuse
program help, community services support, county program aid, and
support through the tribal assistance program.
Angela Identifies clients and families who are at substance abuse risk
and in need of supportive services and will provide referral to
appropriate agencies. She will also provide input, as appropriate, to
help plan substance abuse prevention activities. She can assist the
patient in the application process for social, medical, dental, or
education service with County, State, and Federal agencies. She also
provides family planning referrals and coordinates parenting classes
with other community agencies.

also able to provide transportation patients for social services or to
court (for victims) when necessary.
